Note:
- Payment to be made by Cheque or DD in favour of ISHKS2013
- Online Registrations can be done by logging on to www.ishks2013.com
- Online Charges at the rate of 5% applicable on the registration tariff
- Residential package only at Hotel Hyatt Regency, Kolkata (Venue). Check-in 26th April 2013 & Check-out 28th April 2013. Both the timings are 12 noon
- Residential package includes Room charges for two days, Registration fee, 2 Buffet Breakfasts, 1 Dinner, 3 Lunches and visit to the trade exhibition area
- PG Students have to provide a letter from the HOD
- Accompanying person should not be a doctor in the same department
- Cancellations before 15th April 2013 get 75% refund (25% will be deducted as Administration Charges)
- Cancellations before 25th April 2013 get 50% refund (50% will be deducted as Administration Charges)
- All refunds will be processed post conference
